General Information
Omega House partners with local hospice agencies in our four-county area to provide 24/7 end-of-life care to terminally ill individuals who need these services at our home.
Please note that we are licensed as an Adult Foster Care home and not as a medical facility. While we do have a staff of trained medical professionals in our clinical area, we provide care under the direction of the hospice agency and their team of doctors and nurses.

Admission Instructions for Families
A phone call or visit to Omega House prior to admission starts the process. We accept referrals directly from hospice agencies, doctors and hospitals. If you are not already affiliated with a hospice agency, we suggest that you check with your physician in order to start the process. Each resident must be working with a hospice agency.
The resident and their representative will be contacted by Omega House staff to gather any further information needed and to assure that we can meet the needs of you and your family before a decision is reached.

Initial Criteria for Admission
- Must be signed on with a local hospice agency
- A DNR (Do Not Resuscitate order) must be signed and on file
Not Eligible for Admission
- Aggressive or curative treatment for their primary disease
- Violent or aggressive behaviors
- Infectious or contagious diseases
- Ventilator support, IV therapies, or Tube Feedings
- Blood Sugar Management
- Dementia, Alzheimer’s Disease and mental health issues will be considered on a case-by-case basis
Once mutually agreed that Omega House is the right choice for you, your hospice agency will arrange for your transportation to our home.
Cost for Services
We do not accept any third-party insurance payments including Medicare or Medicaid. Daily rates are based on household resources. Residents with limited household resources may be eligible for 30 days of free care from the Gerald Vairo Memorial Fund. Veterans meeting criteria may be eligible for complete coverage of daily rate.

To begin the referral, fax the following information to 906.482.6144
- Face sheet with demographics
- Medical history with current notes
- DNR
- Current medication list
- DPOA name and contact information
- Referral sent to Iron Mountain Veteran Administration if applicable
